The "Transform Project" uses quantitative and qualitative data-based approaches to identify health care gaps in Illinois and propose potential solutions to ameliorate identified gaps in ambulatory care access and to address important disparites with attention not only to health service delivery, but also structural and environmental inequities related to social determinants of health.
The findings of our Phase 1 report were presented to the Illinois State Legislature and led to multi-million dollar transformation approaches that are currently being mobilized.
